Library Usage Survey - Student/General Public - 2008

Bay Mills Community College Library's Team would like your input on library and computing services. We'd appreciate your completion of this survey as it will help us in our evaluation and planning of services. Thank you for your assistance as we seek to improve services here at Bay Mills Community College Library.

Please select the choice that corresponds to your response to each item. If you wish to state any additional comments about the library which are not covered on this form please email us at library@bmcc.edu and add any additional comments on the items related to the BMCC Library or computing issues. IF YOU HAVE NO EXPERIENCE WITH A SERVICE, PLEASE SELECT - NO OPINION. If you are undecided or have had mixed experiences, please mark “neutral.” The seventh choice on the list is intentionally left blank. Please do not select this option.

Each year the library offers some prizes as incentives to library users to fill out this survey. This year the library is offering a $20.00 gift card for Wal-Mart to three lucky people who complete the survey (there will be only three prizes and they will be chosen from all the respondents - both, student, faculty, or general public. The drawing for the gift cards will take place on April 11th 2008. In order to identify your entry for the gift card please type in a five digit number that only you know where the survey asks for a name. We do not want actual names. It is best if the surveys remain anonymous. Type in a number such as 12345 (don’t use this combination) and at noon on the 11th of April the library will post the lucky numbers of the winners on the entrance doors to the library. To verify this is indeed your lucky number you also need to let the library know what the date was you sent the survey to us.
